Reading Elbow and Hip Scores Before Breeding

What registries record for elbow and hip screening, how to read a grade, and what to check before breeding or buying a dog.

A registry score is a recorded opinion about a specific joint on a specific radiograph, not a verdict on the dog. Elbow and hip programs assign a grade or a category, publish whether the dog passed or failed, and keep the film or scan on file so a later reader can compare. Before breeding or buying, the useful move is to read the grade together with the registry, the age at testing, and the dog's relatives, because the same number means different things across systems.

What a registry database actually records

A certification body stores four things for each dog: identification, the date and age at examination, the examiner's finding, and the registry's own category. Identification is usually a tattoo, microchip, or registration number, which is what makes a record traceable rather than anecdotal. The finding is the raw reading, for example a fragmented coronoid process or an ununited anconeal process on an elbow film, or a subluxation measurement on a hip film. The category is the registry's translation of that finding into a grade, a score, or a pass or fail status.

Most databases also record the method. Hip schemes differ in whether they use a subjective grade, a numeric score, or a measured distraction index. Elbow schemes differ in whether they rely on radiographs alone or add computed tomography, and in whether they report a single grade or a set of primary lesions. A database that mixes methods without labeling them is hard to use, which is why the better resources state the protocol and the age of testing alongside the result.

How do you read an elbow grade?

Elbow grades in the international working group system run from 0 to 3. Grade 0 means no evidence of elbow dysplasia on the submitted study. Grade 1 is mild, grade 2 is moderate, and grade 3 is severe. A dog with grade 0 is commonly described as clear, though clear is a statement about that examination, not a guarantee about the dog's genes or its future joints.

Two details change the reading. First, the age at examination. A preliminary screening in a young puppy can look clean and still change as the dog matures, because the growth plates are open and secondary arthrosis has not yet had time to appear. Second, laterality. Elbow dysplasia is frequently bilateral, and a dog graded on one elbow only has an incomplete record. A grade of 1 in one elbow and 0 in the other is not the same animal as a 1 and a 1, and a database that reports only the worse elbow hides that difference.

The named conditions behind the grades matter too. Fragmented coronoid process, ununited anconeal process, osteochondritis of the humeral condyle, and joint incongruity are the lesions examiners look for. A grade is a summary of those findings, so two dogs with the same grade may carry different lesions with different implications for lameness and for offspring.

What does an OFA or equivalent hip score mean?

Hip schemes fall into two broad families. Subjective schemes ask a reader to place the radiograph into a category such as excellent, good, fair, borderline, mild, moderate, or severe. Numeric schemes assign a number to each hip, where a lower number is better and the two hips are reported separately. Measured schemes estimate laxity directly and report a distraction index, which is a different kind of number again.

Because the families are not interchangeable, a score cannot be compared across registries without conversion rules, and those rules are approximate at best. A fair hip in one system is not automatically a fair hip in another. What a buyer or breeder can compare is the direction of the record: the dog's own result, the results of its parents and siblings, and the proportion of tested relatives that fall on the better side of the scale.

Age at testing matters here as well. Hip laxity can be assessed earlier than some elbow changes, and a preliminary hip reading in a young dog is a screening tool, not a final certification. A registry record that lists the date of examination lets a reader see whether the dog was tested once at eighteen months or repeatedly over several years.

Which numbers should a breeder weigh?

A single clear score is weak evidence. The stronger pattern is a dog with a clear result, tested at an appropriate age, with parents and littermates that also have recorded results, and with a breed context in mind. Elbow dysplasia and hip dysplasia are both heritable but not simple, meaning that many genes and environmental factors contribute, and that two clear dogs can still produce an affected puppy.

Prevalence differs by breed. Labrador retrievers and German shepherds appear frequently in the literature on elbow disease, and breed clubs often publish their own statistics. A breeder comparing candidates can look at the breed's recorded prevalence and ask whether a proposed pairing is likely to move the average in a better direction or simply repeat it.

Practical checks before a breeding decision include: the registry that issued each score, the method used, the age at examination, whether both joints were reported, whether the dog has been re-examined, and whether the pedigree shows repeated clear results across generations. A dog with no record is not the same as a dog with a bad record, but it is also not the same as a dog with a good one.

What should a buyer ask for?

A buyer can ask for the registry name, the certificate number, the date of examination, and the grade or score as printed, not as summarized. A verbal clear is not a record. If the seller provides a certificate, the buyer can check the number against the registry's public search, where one exists.

It also helps to ask what the score does not cover. An elbow grade says nothing about the heart, the eyes, or the temperament. A hip score says nothing about the spine. Screening is joint by joint, and a complete picture of a breeding dog usually includes several separate examinations from several separate programs.

Finally, a buyer can ask about the puppy's relatives rather than the puppy alone. A litter from two clear parents with a deep record of clear ancestors is a different proposition from a litter from two clear parents with no recorded ancestry, even though the parents' certificates look identical on paper.

One may have had a single elbow view, the other a computed tomography study. The grade is the visible part; the protocol is the part that determines what the grade can support.

This is why a database that records method, age, laterality, and the underlying lesions is more useful than one that records only a final category. For a breeder, the goal is not to find a perfect score but to understand what the score was built from, and to make a decision that a reader ten years later could still follow.
